Registration

Instructions for Registering:

  • Complete the form on the following page
  • If playing a CYM sport (grades 5-8 and some 4th grade participants) you must ALSO register with CYM
  • Print out and sign the CYM Parental Consent form showing that your affiliation is with St. Elizabeth
  • Write separate checks for you registration fee and uniform deposit
  • Submit the CYM Parental Consent Form and all monies at the preseason meeting or your at your first tryout or practice
  • Athletes cannot participate until the CYM Parental Consent Form and all monies are submitted (this is a strict CYM and St. E Athletic Association policy)
